 Referrals
New Jersey state law now allows for direct access to a licensed physical therapist. This means that you can see a physical therapist without a prescription from your medical doctor and are not required to see him or her before starting physical therapy. However, some insurance companies still require a written referral from your physician for physical therapy visits to be covered. You will need to check with your insurance company to find out if a referral is required with your plan. In general, if you do not need a referral to see a specialist, then typically you will not need a referral for physical therapy. Please note that we have MOVED!!

Appointment Please call our office to make an appointment (973-895-9925). We will ask for your insurance information, so please have your insurance card handy. The receptionist will schedule your initial evaluation, which usually lasts about one hour. Your First Visit Please bring your insurance card and photo ID, as well as any test results, X-Rays, or prescription you may have received from your doctor. You will be asked to fill out some paperwork when you first arrive, so please come 10-15 min early. Promptness assures the therapist will be able to spend the most amount of time with you on your first visit. If you have the means to, you can download the necessary forms found below, print them, fill them out, and bring them with you on the day of your evaluation. Arm Forms

Neck Forms  One can expect the first visit to last about an hour. We ask patients to wear loose-fitting clothing so your therapist can easily view your injured body part. Please wear sneakers or shoes with good traction to safely use the gym equipment. We have a private locker room if you need to bring a change of clothes, but you will need to bring your own lock.
